Please read our clinic Policy Carefully

Innisfil Endoscopy Suite has been set up to provide medical care for residents of Innisfil and surrounding areas. We are primarily accepting new referrals.

If you would like to make an appointment with one of our specialists, please have your family physician complete a referral form and have their office send it in to us via fax. Once your referral is received, Innisfil Endoscopy staff will examine the referral form to ensure that you are eligible to have an endoscopic procedure in an out-of-hospital facility. Once everything has been looked over and approved our administrative staff will contact you directly to set up an appointment time that is convenient for you.

If you do not have a family physician, but would still like to book your screening colonoscopy please print out a copy of our referral form and take it to a walk-in clinic for a physician to complete the referral for you.

Documentation Requirements

All prospective patients are required to complete an intake form to register and present a valid OHIP card on all medical visits. If your health card is invalid or expired you will be charged at the time of the visit.

No-Show Policy and Lateness Policy

We understand that patients might have conflicting schedules after making an appointment. 

If for any reason you need to cancel your scheduled appointment,  Innisfil Endoscopy will require a minimum of 5 business days’ notice. Failure to comply with this, will result in a $250.00 no-show fee. Patients will not be able to reschedule an appointment until outstanding fees have been paid.
