If prospective patients would like to make an appointment with one of our specialists, they are required to have their family physician complete a referral form and have their office send it to Innisfil Endoscopy Suites via fax or email. Once the referral intake form is received, our clinical staff will examine the referral form to ensure that you meet the eligibility criteria for an endoscopic procedure in Innisfil endoscopy.
Once the referral intake form is reviewed, our support staff will contact the prospective client directly to set up an appointment time.
If you do not have a family physician, but would still like to book your screening colonoscopy please print out a copy of our referral form and take it to a walk-in clinic and ask the physician on duty to complete the referral for you.
